Welcome to Missouri S&T Student Health Services.
Our goal is to provide for healthy students and a healthy campus population. There are certain requirements that need to be met before you can complete enrollment and move onto campus. Failure to meet these requirements will result in a hold on your registration process.
To submit your information, please go to The Student Health Portal and use your Missouri S&T username and password.
Go to the “My Forms” section to read and complete:
Please enter as many immunizations as you can. At a minimum, you must have:
The Meningitis vaccination is strongly recommended for all university students. For adolescents who receive the first dose at age 11 through 15 years, a one-time booster dose must be administered. Adolescents who receive their first dose of Meningitis vaccine at or after age 16 years do not need a booster dose. You will not be allowed to move into campus housing if we have not received your Meningitis immunization documentation.
There are two Meningitis vaccines available, the Quadrivalent (Menactra, Menveo, Menquadfi, or MCV-4) and Meningitis B (Bexsero, Trumenba). The required vaccine is the quadrivalent (Menactra, Menveo, Menquadfi, or MCV-4). The Meningitis B vaccine is only recommended, not required.
More information about Meningitis disease and vaccine can be found here: Meningitis Fact Sheet 2016 and Meningitis B Brochure
Once you have completed your immunization history form, go to the “Document Upload” section of the portal and upload a copy of your immunization record. The specific date that these vaccines were given must be legible on your scanned copy.
Once your immunization record, health history, and TB screening form have been reviewed by a student health nurse, your holds will be removed and campus housing will be notified. This may take up to 5 days.
Other things to consider before coming to campus:
Will you be under the age of 18 when you arrive to S&T? If so, your parents will need to fill out Consent to Treat a Minor before we can see you at Student Health.
Do you have ADHD? If so, please review the ADHD Policy, and make a plan with your parents before you arrive at S&T
IF YOU NEED ASSISTANCE WITH THE PORTAL, PLEASE CONTACT STUDENT HEALTH VIA EMAIL OR BY PHONE.
