1. Measles, Mumps, Rubella vaccine x 2. The first one after 12 months old; second @ least 30 days later.

2. Meningitis vaccine OR waiver: Required to live in University housing.

3. Health History Form: Essential before we can see you as a patient.  

4. Tuberculosis screening: If you were born or lived >3 months in an endemic area.
