We can verify a student visit at the Health Center for an acute illness or injury being treated at the Missouri S&T facility.

The student will need:
To be present when the verification is written.
Will need to sign that he/she has requested and authorized the release of information.
Will be responsible for delivering the document.

Student Health
Will only document that the student was in the health center for an illness or injury.
Can not release any details of the visit to include diagnosis and treatment.
Will time and date the note for the day of service only (no prior or future days).
Does not have the authority to exempt students from classes or exams.
Strongly encourages students with contagious disease to contain their illness.
